Individualized Strength Based Treatment Planning
Upon admission, each patient is assigned specific goals to work on based on their diagnosis and presenting problem(s). At admission, an individualized strength based treatment plan is developed by the multi disciplinary team with input from the patient, family members and referral source. This treatment plan is reviewed and updated monthly.
Daily Treatment Schedule
All days and activities are scheduled in advance and reflect the necessity of daily opportunities to work on all treatment areas.
All patients are expected to complete Community Restoration to help make amends and restore the impact their actions have had on the patient, the patient’s victims, and their individual communities. Community Restoration is one tool that is used to assist the youth in beginning to take responsibility and ownership of their actions, relationships, and communities.
Opportunities to complete restoration hours are built into the daily schedule.
Additional unit outings and privileges available to upper level patients require that weekly community restoration hours be completed.
A patient is required to complete victim restitution for usually one of two main reasons; the patient’s referral source has a court order that requires a set amount of restitution hours be completed or restitution will be ordered when a patient has damaged hospital property while in treatment.
If a patient owes victim restitution they are required to complete four hours per week in order to qualify for any additional upper level privileges.
Program Outcome Measures
Benchmark has developed a unique method of measuring patient progress in treatment. Measures are utilized in communicating progress to patients, parents and referral sources in a quantifiable manner.
Data is collected weekly and analyzed by the treatment team on a monthly basis. The analysis is used to adjust the patient’s individualized treatment plan and can be used to determine discharge readiness.
