Admissions to Benchmark Behavioral Health

To speak to our referral staff, call 801-299-5319. We are here to answer your questions and guide you through the admissions process.

In order to begin our admissions process, our Admissions Coordinator will ask for a clinical packet, which should include:

  • A recent psychiatric evaluation
  • Documentation of any psychological testing
  • A treatment plan from current placement or discharge summaries from prior treatment
  • A birth certificate
  • Documentation regarding any medical issues the client might have
  • Social security card
  • Medicaid card, insurance information (if applicable)
  • Individualized Education Plans, (IEPs)
  • Any additional information that would help our clinical team determine eligibility

Our clinical team and physicians will review the completed packet and will determine admission. The referring party will be notified of acceptance or denial if the individual is not appropriate for our program.

If accepted, an Admissions Coordinator will work with the referring party to finalize the admission.

Admissions Criteria

Each potential client can have a variety of misconduct issues but at least one type of psychiatric disorder has to be among them. Benchmark Behavioral Health provides treatment for a variety of psychiatric and behavioral disorders including:

  • Conduct disorder
  • Sexual disorders/sexual misconduct issues
  • Fetal alcohol spectrum disorders, Asperger’s disorder
  • Developmental disorders
  • Mood disorders
  • Anxiety disorders
  • Personality disorders
  • Substance abuse issues

We have created a list of our basic qualifications to give an idea of what clients might be an appropriate choice to participate in treatment at Benchmark.

Basic Criteria for Admission:

  • Males
  • History of sexual misconduct
  • Poor anger management/impulse control
  • 13-17 years of age for clients with Sexual & Behavioral Misconduct issues
  • 18-20 years of age for clients with only Sexual Misconduct issues
  • IQ of 50 or higher
  • Substance abuse
  • Adjudicated and non-adjudicated

Insurance Information

There may be circumstances where a client has insurance in addition to or other than state Medicaid. If so, this information must be provided to the facility upon request for admission into one of our programs. All insurance will be verified for eligibility and benefits.
